Disability Forms & Documentation — $150
Completion of short-term disability, long-term disability, or other disability-related documentation is available for established therapy clients only.
Clients must have completed a minimum of 6 therapy sessions with Unified Counseling before disability forms or supporting documentation can be considered.
This allows sufficient time to assess symptoms, functioning, treatment progress, and the clinical information needed to complete the paperwork appropriately. Completion of six sessions does not guarantee that disability documentation will support a specific leave period, restriction, accommodation, or disability determination.
Fee: $150 for standard disability documentation.

Subpoenas, Court Orders & Required Legal Involvement
Unified Counseling does not routinely participate in voluntary court appearances, depositions, or attorney meetings. If records, testimony, a deposition, or a court appearance is legally required through a valid subpoena or court order, the request will be reviewed individually. Applicable fees for preparation, record review, travel, waiting time, testimony, or other required professional time will be communicated when appropriate and permitted by law.
Medical Records
Medical Record Requests — Fees Vary. Requests for copies of medical records are processed in accordance with applicable federal and state law. Any permitted copying, preparation, media, postage, or other applicable charges will depend on the format and scope of the request. Clients will be informed of any applicable fee before records are released when required.
Psychotherapy notes that are maintained separately from the designated clinical record are handled differently from ordinary patient-access requests.
Expedited Documentation
Expedited service is subject to availability and the complexity of the request. Rush fees do not guarantee that a document can be completed by a deadline that does not allow sufficient time for an appropriate clinical review.

Important Information
Evaluation and documentation services are based on independent clinical assessment. Scheduling or paying for an evaluation does not guarantee a particular diagnosis, recommendation, accommodation, letter, surgery clearance, immigration outcome, disability determination, leave period, or other requested result. Clinical recommendations are made based on the information available and the clinician’s professional judgment.
Clients should confirm the specific documentation and credential requirements of their physician, surgeon, attorney, employer, school, insurance company, or other requesting organization before scheduling. Evaluation and documentation services are generally self-pay unless insurance coverage has been confirmed in advance. See our fees and payment page and terms and policies.
Frequently Asked Questions
Are evaluations completed virtually?
Yes. Available services are provided through secure telehealth when clinically appropriate and permitted in the client’s location.
Does paying for an evaluation guarantee a letter or favorable recommendation?
No. Evaluations and documentation are based on independent clinical judgment. A particular recommendation, diagnosis, accommodation, or outcome cannot be guaranteed.
Can you complete forms for someone who is not a therapy client?
Some evaluation services may be available to non-clients. Ongoing-treatment forms, such as disability documentation, may require an established treatment relationship. Disability forms require a minimum of six completed therapy sessions.
How quickly will I receive documentation?
Turnaround depends on the type and complexity of the request. Rush service may be available for an additional fee.
Will insurance pay for evaluations or paperwork?
Many administrative, forensic, legal, and documentation services are self-pay. Insurance coverage varies and should be confirmed before scheduling.
Can you send documentation directly to my provider, attorney, employer, or school?
Yes, when appropriate and after the necessary written authorization has been completed.
